Common Breastfeeding Problems
The NHS Clinical Knowledge Summaries are a reliable source of evidence-based information and practical 'know how' about the common conditions managed in primary care. This 'quick' resource for health professionals includes assessment and management of sore nipples, painful breastfeeding and low milk supply
Assessment of problems Problems when establishing breastfeeding can be identified using this UNICEF Breastfeeding assessment form
Use of drugs to initiate or augment milk supply Because an apparently low milk supply is one of the most common reasons for discontinuing breastfeeding, both mothers and clinicians have sought means to address this problem. Galactagogues (drugs for faltering milk supply) should only be used after thorough evaluation for treatable causes such as poor attachment, and when increased frequency of breastfeeding, pumping or hand expression of milk has not been successful. In Greenwich mothers should be referred to a breastfeeding group and/or health visitors for additional support with feeding. Domperidone is the drug of choice because of its adverse effect profile and efficacy, and because only small amounts pass into breast milk.
